This page is designed to provide you with all needed registration information and required forms for the upcoming 2016-2017 school year.
To register your child, you must bring all needed forms and required documents to Narcoossee Middle in person.
Rules of Enrollment for 2016-2017
See attached "Rules of Enrollment" document for full details.
I. Proof of Residency in School Zone:
A resident parent or guardian admitting a Student to an Osceola County School shall produce two documents from each of the following categories:
Category 1:Mortgage document, rental or lease agreement, property tax records
Category 2: a.Current utility bill including cell phone,electric, cable or water (a 30 day waiver is granted when the family is just moving into the address for this provision. A date should be attached to this waiver providing information as to when the utility bill should be presented.)
b.Income tax records
c.Proof of receipt of government benefits
In the event the parent or guardian is not the lease holder or property owner the following is required:
A completed, notarized Verification of Residency available through Student Services Department located at the district office signed by the owner of the home with one document from each of the above categories. In addition, parent must provide Declaration of Domicile,valid Florida Driver’s License (or DMV picture ID), voter registration or auto registration with the current Osceola county address listed for which they reside.
II. Proof of Physical Examination:
The physical examination must have been completed within the last twelve months prior to the anticipated day of enrollment by a health professional who is licensed in Florida or in the state where the student resided at the time of the exam (6A-6.024 F.A.C). If the student has previously been in a Florida school a thirty (30) day exemption will be given while waiting for previous school records. If, when requested records are received, or upon other verification as documented, no valid physical examination documentation is presented,the student will be required to submit to a physical examination.Students entering Kindergarten from an Osceola County School District VPK program will have already met the school entry physical requirement
All student immunizations must be up to date and on Florida Form 680.
Grade 7-12 All required immunizations and a Tetanus (Tdap) booster***
***Students thirteen (13) years of age and older who are required to have Varicella for school entry should received two (2) doses at least twenty-eight (28) days apart of the Varicella vaccine if they have not already gotten the shot or had the disease.
IV. Proof of Date of Birth:
State law requires that a child must be five years old on or before September 1 of the ensuing school year to be eligible to enroll in kindergarten. A child must be six years old on or before September 1of the ensuing school year to be eligible to enroll in first grade. Such proof can be provided with one of the following documents:
a. Birth certificate (certified copy), or
b. Certificate of baptism, with sworn affidavit,
c. Two-year old life insurance policy on child, or
d. Bible record, with sworn affidavit, or
e. Passport showing age, or
Affidavit of age sworn to by parent and a certificate of age signed by health officer, school physician or licensed physician.
